How To Prepare For Your First Appointment

During your first visit with us, you will meet with our doctor and staff. We will collect information about your medical history at this time to help us determine how best to assist you, starting with what testing needs to be performed. Some testing may take place on your first visit, and the doctor will discuss all options and testing procedures with you.

We recommend filling out the forms below prior to your first appointment in order to save you time at our office. If you are unable to fill out and file forms before the day of your appointment, please arrive no less than 30 minutes before your scheduled appointment time in order to provide necessary information. This information is critical in ensuring safe, effective treatment of our patients.

Please fill out the forms prior to coming to your appointment as they are extensive and will become part of your medical record. You may e-mail it or fax forms to our office.

Please be aware that for each appointment we book, we reserve that time slot especially for you and therefore have to run our schedule on time. Kindly give us a 48-hour notice to cancel or reschedule your appointment, otherwise you will incur a cancellation fee. You may cancel on ZocDoc, online on our website, or over the phone.

During Your Visit, Please Bring The Following:

  • Identification
  • Insurance Card
  • A List Of Current Medications You Are Taking
  • The Name Of Your Referral Provider

We also ask that out of courtesy to other patients that you refrain from smoking prior to your appointment or wearing perfume/cologne into our office.

Prior To Your Appointment

Please Also Confirm Your Allergy Benefits With Your Insurance Provider Before Your Appointment.

With all the recent changes with health insurance we ask for your assistance in confirming your insurance coverage to ensure that there are no surprise out of pocket expenses for your treatment.

Physician Referral – Do You Need It?

We often find that referrals are required for allergy and asthma treatments. Your health insurance company can inform you if a referral from your Primary Care Physician is needed prior to making an appointment with us. If so, please make sure to get this referral prior to scheduling an appointment with us to save you time and headache.

Know Your Coverage

We understand that typically medical practices check the coverage but with the continual changes with health insurance, We Ask Our Patients To Help Out With Confirming Their Plan Is In Network With Dr. Laura Ispas And To Verify Allergy Benefits And Coverage For Skin Testing And Allergy Shots.

We find that our patients are the most satisfied with their appointments and treatments when they know ahead of time what their health insurance plan covers and what type of out of pocket expenses they may incur.

If You Are Getting Skin Testing Please DO NOT Take Antihistamines (Zyrtec, Benadryl, Zantac, Pepcid, Allegra, Etc.,) For 72 Hours PRIOR To The Visit.

If you have any recent medical records such as office visits, labs, and/ or Emergency Room visits, please bring those with you to your appointment.

In Case Of Emergency

If You Are Suffering From Anaphylaxis, An Asthma Attack, Or Other Emergency Situation, Call 911 Or Visit The Emergency Room Immediately.
